Physicals and Immunizations Needed

Per Illinois School Code requirements, all Kindergarten and Sixth graders need a physical exam completed within the past year, along with up-to-date immunizations. If you haven't turned in your child’s completed physical and immunization forms yet, please drop them off at the main office as soon as possible.
